Food Questionnaire and Kitchen Form

The Food Questionnaire is needed to create menus around your food preferences and also to determine what foods to avoid should you have food sensitivities.

We can review both these forms together during the consultation or they can be completed ahead of time. It's up to you.

The Kitchen Form is used to jot down what equipment and gadgets you have available for the cook day outside of the basic tools.

For example, if you have a dutch oven cooking pot with a tight fitting lid, the list of melt-in-your mouth braised dishes available to cook is endless.
